Answer  It is a snug fit with the standard battery.

How easy is it to take the phone out of the pouch? Some other models have push holes. Does this one? Does the pouch cover the speaker on Droid 4?

Answer  I have an Otterbox Commuter case on my Droid 4 and I couldn't use the Noble Stiched Horizontal Leather Pouch because it was too tight. I could force it in, but it was nearly impossible to remove it was so tight..

When using the belt loop, can you remove the clip?

Answer  If you pick up (open) the clip you can find a small keeper at the bottom of the slide-in part of the clip. If you push this in you can slide the clip off the case. The clip has a hook on the bottom that fits over the bottom of your belt and should help keep it on with your belt. I use both the clip and the loop and it's worked well for the two days I've had it.
